Individualized Education Program (IEP): Transition Planning

Transition planning is a crucial component of the IEP for students with disabilities, beginning no later than when the student turns 14.


The plan must include:


  • Post-secondary Goals: Clear, measurable goals for life after high school, including education, employment, and independent living.


  • Transition Services: Specific services and activities designed to help the student achieve these goals.


  • Annual IEP Goals: Objectives that align with the student’s transition goals and help them make progress toward post-secondary success.
