Transition planning is a crucial component of the IEP for students with disabilities, beginning no later than when the student turns 14.
The plan must include:
Post-secondary Goals: Clear, measurable goals for life after high school, including education, employment, and independent living.
Transition Services: Specific services and activities designed to help the student achieve these goals.
